Clinical Decision Support for Quality Measures
Efficiently record and report on CMS physician quality measures and/or The Joint Commission hospital core measures to expedite clinical priorities and maximize pay-for-performance financial reimbursements. Wellsoft’s quality measures mechanism utilizes a single table strategically placed on the tracking, triage, and chart displays to remind the triage nurse and the physician to record and perform patient applicable quality measures. When utilized on the tracking display, the Quality Measures Management tool will allow Emergency Departments to define custom "rules" that scan multiple areas of the patient record and perform conditional statements to identify all “Quality Measures” patients in real time and display the elapsed time since the care considerations were initiated, and present color codes that reflect their completion status. Three Reports – a Quality Measures Log, Quality Measures Discrepancies Report, & Quality Measures Tally – are provided to generate performance reports based on the data collection table to promote superior quality of care.
The Core Measures Wellsoft currently supports are:
- Aspirin on arrival for Acute MI
- Beta blocker on arrival for Acute MI
- ECG performed for pt w/non traumatic chest pain
- ECG for Syncope
- Vital signs for community-acquired bacterial pneumonia
- Oxygen saturation for community-acquired bacterial pneumonia
- Mental status of patients with community-acquired bacterial pneumonia
- Empiric antibiotic for patients with community-acquired bacterial pneumonia
Split Screen View
This new feature significantly enhances the amount of patient information and status of the Emergency Department presented to the user. Each Split Screen view is interactive and can be configured to display information independently. For example, in a vertical side-by-side Split Screen view, the left half of the screen can be configured to display the Patient Track showing solely patients that are currently in beds in the Emergency Department. On the right half of the screen, the Patient Track can be configured to display only the beds still available in the Emergency Department. As patients are moved into and out of rooms, the Split Screen views reflect the changes dynamically. The Split Screen feature can be configured to present quadrant views of the Emergency Department as well.
Display Unoccupied Rooms
Wellsoft can now be configured to display unoccupied rooms on the Patient Tracking Display. With this new feature, Wellsoft can display both occupied and empty rooms together directly on the Patient Track Display. This new capability works in conjunction with the existing sort and filtering of the rooms displayed on the Patient Track Display, presented by Area of Care, Districts, Pods or for the entire Emergency Department.
Patient Entry Kiosk/Display
The Wellsoft Patient Entry Display will allow patients to enter their own names, chief complaint, sex and age into the Wellsoft application through a user friendly graphical interface. Entering their names will populate the triage track with their information once processed.
Professional Charge Capture & Coding
Faster and more efficient billing is achieved using our automatic professional charge capture and coding feature. Wellsoft EDIS will automatically determine the E&M level of service for a particular patient visit based on physician documentation, procedures, orders, results and final diagnosis. Physicians will have the option of viewing their current E&M level at anytime during a patient’s visit. This summary view will include a breakdown of all the elements (HPI, PFSH, ROS, Physical Exam, Decision Making) contributing to the E&M level of service. All billing codes (CPT, HCPCS, APC and ICD-9) will also be populated as soon as a diagnosis is determined, a procedure is documented or an order is placed. All chargeable items and codes can be displayed, printed or sent via HL7 interface to a billing system. This process enables the hospital to generate a more consistent and objective professional level bill, supported by the medical record.
Risk Management System
The Wellsoft EDIS Risk Management System introduces the ability to implement complex risk management checks and flags during a patient’s visit. Examples of current rules built into the system include medication alerts based on a patient’s age and weight, warning messages for abnormal vital signs, as well as specific PQRI measure alerts that can warn you, for example, if your “Pneumonia patient over 18 years old hasn’t had an O2 Saturation taken yet."
Patient Photo ID / Photo Capture
View patient image data at the point of care using Wellsoft’s user-friendly photo capture utility. Patient images can be imported from attached webcams or rapidly transferred from digital cameras or flash storage media. This functionality will be useful in patient identification, risk management, and wound or abuse cases, among others. For patient identification, the image can be placed in the patient summary bar at the top of each record. This image is visible on the Triage Display, Chart Display and any other patient record display. Wound, or any other body images, can be easily embedded in the clinical documentation.
Wellsoft’s Image Editor allows end-users to quickly enable a webcam, take or erase a photo, zoom in or out, browse through all images within a particular record or print thumbnail or full-sized images.
Anatomical Diagrams
Wellsoft EDIS incorporates detailed anatomical images which can be embedded within clinical documentation. These easily accessible images can be annotated with a mouse, stylus, or touch screen to indicate lacerations, edemas, etc. Clinical findings can be included above or below an image thumbnail if there is a need to expound upon any annotations.
Wellsoft’s Image Editor allows end-users to quickly change a particular diagram’s orientation from left to right, zoom in or out, browse through all images within a particular record or print thumbnail or full-sized images.
Scanned Documents
Reduce the costs associated with storing and duplicating paper documents and increase the accessibility of your documents by scanning them directly into patient charts. Wellsoft EDIS allows for the scanning of both images and paper documents, such as EKGs and EMS run sheets, ID cards, or forms. Scanned documents can be viewed as thumbnails or full-sized images, and similarly printed in either format.
Embedded Forms
Wellsoft now provides the ability to reproduce forms electronically, replacing your facility’s standard paper forms with digital conversions that are stored, accessed and electronically signed within Wellsoft EDIS. The electronic signature can be permanently embedded in the patient record and secured by our comprehensive user access system.
Sketches Signatures
The annotation functionality in Wellsoft EDIS allows clinicians to sketch over anatomic diagrams for the visual documentation of lacerations, burns, skin lesions, masses, etc., incorporate free-form drawings on an integrated “sketch” pad, or capture electronic patient signatures. Input devices include mouse, stylus, or touch screen.
Care Plans / Special Patients
Store a longitudinal record of patient specific protocols for special patients, typically those with chronic issues, such as sickle cell anemia, migraines, and pain management. Links notify the user upon registration of an identified special patient. Wellsoft maintains individualized “care plans” for these patients enabling physicians to maintain continuity of care by being informed of the patients’ history of treatment and any other pertinent information.
Integrated Medication System
including Drug Dosing Calculator
Automatic weight-based dosing calculations are now integrated into Wellsoft EDIS including alerts based on minimum/maximum doses. A significant risk management enhancement, Wellsoft provides a medication display that works with orders and/or prescriptions.
Medication Reconcilation
Decrease the possibility of wrong drug and wrong, duplicate or missed dosage errors using the Wellsoft EDIS medication reconciliation feature. Designed to enhance patient safety, an expanded and enriched medications table includes a source column to capture the reliability of medication information. An MD Review column has been added to document home medications instructions. The patient’s home medications instructions are then printed along with prescriptions if discharged and with orders if admitted.
Drug/Drug Interaction Checking
Medications ordered, prescribed, or dispensed in the ED can now be checked automatically or on demand against medications taken and known drug allergies for possible drug-drug interactions or allergy/hypersensitivity reactions. An integrated link in the Wellsoft system to Gold Standard, a leading developer of drug information databases, provides immediate, up-to-date, clinically-relevant information on all U.S. prescription drugs, as well as off-label uses and dosage, herbal supplements, nutritional and over-the-counter products and new and investigational drugs.
Pharmacy Orders
Pharmacy orders, including repeat orders, can now be sent electronically to the hospital pharmacy system using Wellsoft EDIS. Each order entered using Wellsoft’s medication display can be sent to the pharmacy system through an HL7 interface (ORM/RXO). The Wellsoft system will also be able to receive inbound HL7 messages (ORM/RXO) and populate the patient’s record with the appropriate medication order.
Radiology Tracking Display
Dynamically track patients with radiology results apart from other patients to enable easy comparison of initial readings with the radiologist’s finding. Track patients by physician or time and flag for further follow up and quality assurance pathways.
